Acid Reflux Part 3 - Laryngopharyngeal Reflux (LPR)

While Laryngopharyngeal Reflux (LPR) is cited in some journals as a part of GERD, it presents its own set of symptoms and potential health problems. LPR affects the larynx, pharynx, bronchi, trachea, and possibly even one's lungs.

LPR occurs when the upper esophageal sphincter (UES) opens and allows reflux to enter the throat. Since the components of the affected area (see the list above) are constructed of softer tissue that the esophagus, the potential for damage is greater from stomach acid and digestive enzymes. And if the reflux is "aspirated," or breathed into the lungs, the risk is increased for one to develop aspiration pneumonia. Other pulmonary problems whish may manifest are coughing, wheezing, asthma (especially adult onset), and interstitial fibrosis (the tissue in the lungs becomes inflamed and/or scarred). Excess acid reaching one's mouth can also lead to the decay of the tooth enamel, gingivitis, halitosis, and waterbrash.

Further complications can include odynophagia and dysphagia. And pharynx symptoms can include laryngitis, hoarseness, soreness, the sensation of a lump in one's throat (globus), and earache due to fluids entering one's Eustachian tubes between the throat and the ear.

Overall symptoms may include the above plus:

- Dysphonia
- Frequent throat clearing
- Post nasal drip
- Spasm of the larynx
- Blockage of the breathing passage
- Swallowed food coming back up
- Difficulty singing, especially the loss of vocal range.

Diagnosis of LPR can be more difficult, due in part to the fact that less than 15% of those affected have the "heartburn" associated with typical GERD symptoms. Your physician or health care provider may do a laryngoscopy to diagnose and evaluate the condition. Please consult him/her as quickly as possible if you experience any of the aforementioned symptoms.

Other testing may include:

- pH testing - two sensors are placed (one on each end of the esophagus) to determine if the reflux is reaching one's upper esophagus and to assess the pH content of the reflux.

- Upper gastrointestinal endoscopy, if one complains of dysphagia or odynophagia

Lifestyle changes:

These are the same as those mentioned in Part 2 of this three-part series.

Medical treatments:

- Antacids - Prescription medications to reduce acid production in the stomach
- the Nissen Fundoplication mentioned in Part 2.

As with treatment of GERD one's physician may recommend one, or a combination, of the treatments cited, depending on the severity of your case. And it is recommended that one seek medical care as quickly as possible, should one experience any of the symptoms described in these articles.

Gastroesophageal Reflux Disease (GERD) occurs when the lower esophageal sphincter (LES) fails to remain closed. There are several theories on why this happens. It is most commonly believed that the tissue structure of the LES is compromised by one or more of these factors:

- Overeating
- Eating foods which contain high acid content
- Delayed digestion of stomach content
- Diminished salivation
- Relaxation of the LES (transient)
- Decreased LES resting status
- Ineffective esophageal clearance
- The inability of the esophagus to resist injury and repair itself

GERD episodes occur frequently during the day, quite often following the consumption of too much food or foods high in fat content. However there are also many reported cases of this happening at night, while one is in a horizontal sleeping position.

In the course of this research, I discovered that most physicians recommend that patients avoid alcohol, caffeine and chocolate, as these items seem to be the biggest "offenders" in one's battle against this affliction. It should also be noted that smoking, tight clothing, drugs, hormones, obesity, pregnancy, and even body position have been cited as triggering factors in the onset of this disease.

GERD is often accompanied by a hiatal hernia. This is a protrusion of part of the stomach through the esophageal hiatus, an opening in the diaphragm, which leads into the chest. When this type of hernia is present it may contribute to transient LES relaxation and delay clearance, due to impaired esophageal evacuation. This means that hiatal hernias may allow prolonged acid exposure after reflux occurs, thus creating GERD symptoms and esophageal damage.

Common symptoms of GERD involving the LES are:

- Heartburn - burning in the chest behind the sternum
- Regurgitation - not to be confused with vomiting, which employs the gastrointestinal and abdominal muscles to force stomach contents up the esophagus, this is a bitter or acid taste in the mouth caused by the gastric contents reaching it during reflux
- Dysfunctional swallowing (Dysphagia) - this is the sensation of food "hanging up", "getting stuck" or "slowing down" between the pharynx and the stomach
- Pain when swallowing (Odynophagia) - a sharp pain beneath the sternum that occurs when swallowing. This pain is caused by erosive esophagitis (when the lining of one's esophagus has been "eaten" by stomach acid and gastric enzymes)
- Chest Pain - when caused by reflux, this may be sharp or dull and may radiate into the neck, arms, or back. This is very often mistaken as a heart attack. Don't take any chances. Seek immediate medical attention in the event that this occurs so that your physician or health care provider may rule out heart problems and help you treat the cause.

Common tests used to diagnose and evaluate GERD are:

- Upper gastrointestinal endoscopy and possibly a biopsy of the esophagus
- pH testing of the LES - a sensor is put into place for 24 hours to determine whether or not the reflux is occurring, and to assess the level of the acid being refluxed.

Depending on your physician's evaluation, GERD may be controlled in the early stages with diet and an over-the-counter antacid. It is not recommended that one employ this method of treatment without first consulting one's physician. More severe diagnoses may require more in-depth treatment, possibly including surgery.

Your physician may recommend one, or a combination, of the following:

Lifestyle changes: - Elevate the head of one's bed 4 to 6 inches (5 to 8 centimeters)
- Antacids to neutralize excess stomach acid
- Avoid overeating
- Avoid alcohol, chocolate, and caffeine
- No food two to three hours before going to bed
- Lose weight
- Avoid fatty and/or greasy foods
- Drink eight 8 ounce glasses (about two liters) of water daily

Medical treatments:

- Prescription medications to reduce acid production in the stomach
- Endoscopic suturing - this is a minimally invasive procedure and can create a barrier to minimize reflux in those with mild to moderate GERD. It cannot be done, however, if one has a large hiatal hernia.
- Surgery on the LES - The most common is the Nissen fundoplication. But, there is a less invasive form of surgery using laparoscopy. Your physician will determine which better suits the necessity.

The term GERD is a acronym for gastroesophageal reflux disease and is a very glorified term for heartburn. Frequent, persistent heartburn is the primary symptom associated with acid reflux disease or GERD. With GERD (gastroesophageal reflux disease), stomach acid flows back into the esophagus, the tube leading from the throat to the stomach. If you suffer from persistent heartburn two or more days a week, you may have gastroesophageal reflux disease (GERD). Though heartburn is the most common of the acid reflux disease symptoms, other signs may also manifest themselves.

For these individuals, acid reflux disease, also known as gastroesophageal reflux disease (GERD), may be at the heart of their burn. Prevacid is a prescription drug that helps GERD-sufferers prevent the acid that causes heartburn pain associated with acid reflux disease.

Pain

Besides the discomfort of heartburn, reflux results in symptoms of oesophageal inflammation, such as odynophagia (pain on swallowing) and dysphagia (difficult swallowing). Remarkably, the frequency of severe heartburn seems to decline with age, possibly due to a decrease in esophageal pain perception and atrophic gastritis. Although elderly patients may have fewer complaints of heartburn, their disease is usually more severe and has more complications. The burning, pressure, or pain of heartburn can last as long as 2 hours and is often worse after eating; and, can include very unpleasant symptoms, such as heartburn, chest pain, trouble swallowing, hoarseness, throat irritation and bad breath. However, some people with GERD may not experience heartburn, but instead feel a pain in the chest, hoarseness, or problems swallowing. The feeling of heartburn my be similar to heart pain, but in actuality a heartburn has nothing whatsoever to do with the heart. Never assume that chest pain is heartburn or ingestion. GERD Symptoms Heartburn is described as a burning pain in the stomach that rises up towards the chest or towards the neck. A positive Bernstein test is considered if the patient experiences heartburn or chest pain. The acid perfusion test may have its utility more in explaining reflux as the cause of atypical chest pain. Potentially dangerous confusion can arise when someone neglects their heart pain believing it to be heartburn.

Gerd

Only 2 to 3 percent of acid reflux events reach the conscious level and are perceived by patients with GERD. Abnormalities that make it dysfunctional promote acid reflux and the constellation of GERD problems. Many people, including pregnant women, suffer from heartburn or acid indigestion caused by GERD. Most cases of acid reflux respond to lifestyle changes and prescription medication. Anyone experiencing heartburn twice a week or more may have GERD. Occasional heartburn is common but does not necessarily mean one has GERD.

Acid reflux occurs when one of two closures, or "sphincters," in the esophagus fail to remain sealed after food and/or liquid pass through them on the way to one's stomach. The two sphincters are located at each end of the esophagus. The first one is at the top, directly behind the larynx and pharynx. The second one is at the bottom, directly above the stomach.

There are two forms of acid reflux - Gastroesophageal Reflux Disease (GERD) and Laryngopharyngeal Reflux (LPR). Either form can cause serious and permanent damage, including some forms of cancer, if not diagnosed and treated on a timely basis. Please read further in Parts 2 and 3 for some of the symptoms, tests for diagnosis and evaluation, and treatments commonly used to control this potentially devastating disease.

Acid reflux is scientifically known as GastroEsophageal Reflux Disease (GERD). It is a disease characterized by the abnormal reflux of gastric contents into the esophagus resulting to chronic symptoms or mucosal damage.

Acid reflux is often caused by temporary or permanent alterations in the lining that separates the esophagus and the stomach. Ineffectiveness of the lower esophageal sphincter (LES) or a temporary LES relaxation connected with a hiatal hernia are just some of the common causes of acid reflux. The process can also lead to a backflow of gastric juices to the pharynx or mouth.

Among adults, the most common symptom of acid reflux is heartburn which is caused by the presence of acid in the esophagus. Heartburn is characterized by a painful burning sensation behind the sternum or breastbone. Another symptom or manifestation of acid reflux is esophagitis which is characterized by an inflammation of the lining of the esophagus which is also known as the mucosal lining. Esophagitis also causes swallowing difficulties and chronic pains in the chest area.

Sometimes, individuals suffering from acid reflux may also experience coughing, persistent pain in the ears, hoarseness or a change in the voice and even sinusitis. If acid reflux gets complicated it may lead to formation of a stricture or ulcers in the esophagus. It may also lead to a condition called Barrett's esophagus and in worst cases, to cancer of the esophagus.

It doesn't mean however that a person who regularly suffers from heartburn is suffering from acid reflux. By all means, the heartburn may be from other causes. But if a person suffers from heartburn for more than once a week, then he or she is at a risk of acquiring acid reflux. Persons with hiatal hernia are also at a greater risk of developing acid reflux than those who are not.

The pain felt by persons suffering from heartburn is caused by the reflux of acid contents from the stomach to the esophagus. A pain in the chest area coming upwards the mouth.

Persons suffering from acid reflux may also experience tasting something sour or salty behind her throat. This is cause by regurgitation. This sour and salty taste may persist even without heartburn,

Other less common symptoms of acid reflux include difficulty swallowing, chest pain, halitosis or bad breath, repeated throat clearing and water brash or hyeprsecretion of saliva.

The symptoms of acid reflux in children are also the same as that on adults. Acid reflux in children may manifest in frequent spitting, throwing up repeatedly, coughing and other respiratory problems. Children suffering from acid reflux may also experience weight loss, frequent crying, loss of appetite and bad breath. Parents must remember though that children may show one or many symptoms. There is not one symptom that is always present in children with acid reflux.

The cause of acid reflux in children, especially in babies is their immature digestive system. This is why infants stop having acid reflux when they reach the first year of age. However, some children do not outgrow acid reflux. Some continue to suffer from the disease up to the teen years.

The best thing to do for parents of children suffering from acid reflux is to take the children to the doctor as soon as possible to avoid any complications.

To counter the surge in the prevalence of acid reflux, lots of over the counter medications have been churned out of our pharmaceutical companies. Yet, acidic reflux disease can be traced to aging in adults. It is believed in some quarters that as we age, the activity of the stomach reduces, so also its ability to produce hydrochloric acid. The reduction in the stomach activity and acid content of the stomach creates a breeding ground for infections which in themselves cause stomach pain and acid production leading to further symptoms of acid reflux.

Whatever the cause of acid reflux disease, what is constant about it, is the fact that it is a chronic disease. Most medicines, which inhibit production of acid in the stomach, only help to relieve the burning effect and other symptoms and not actually cure the condition. As a matter of fact, these drugs help in relieving the symptoms quite effectively, but the other side of the coin is that these drugs also cause several side effects in the body by cutting down the acid production in your stomach. Some of their side effects are that they reduce the ability of the stomach to digest food efficiently, they make you more vulnerable to diseases and microbes transmitted through food, increases the risk of food poisoning and a host of others.

While there are many medications and natural methods to rid yourself of that burning sensation that accompanies each acid reflux bout, the first line of action should be a thorough assessment of the your food and lifestyle. Some food materials are known to aggravate acid reflux, some of the foods include; citrus, caffeine, chocolate, fatty fried foods, garlic, and onions. Cutting down or avoiding these foods as much as possible is a good first attempt at preventing future acid reflux bouts.

Additional lifestyle assessment and modification that can help reduce acid reflux episodes include:

- Losing weight if you are overweight
- Cutting down alcohol to the barest minimum
- Eating just enough food to keep hunger away
- Don't go to bed until at least three hours after eating
- Raise your bed head six to eight inches

These very simple lifestyle improvements can keep you off those acid blockers and other acid reflux medications that cause almost the same amount of damage as the good they do to your body.

Some medical conditions have no respect for age; the young or the old, and Acid Reflux, also known in medical terms as Gastroesophageal Reflux Disease (GERD) seems to be one of them. The lower end of the esophagus is protected by a valve called the Lower Esophageal sphincter (LES). This valve opens to allow food from the esophagus into the stomach and then closes up to protect the esophagus from the acid content of the stomach. Several conditions can compromise this valve, allowing gastric content into the esophagus. As in adults, infants also suffer from heartburn on occasions. This is mainly attributed to the fact that infants consume essentially liquid and soft foods, which tend to be rich in diary proteins. The most unfortunate part of infant acid reflux disease is that, unlike in adults, it is very difficult to determine if an infant has developed chronic gastroesophageal reflux disease. An infant is most unlikely to be able to complain of heartburn or any symptoms of the disease.

Acid reflux in infants is always as a result of a lot of factors, however, most of the causative factors tend to be aggravated by the fact that infants, spend a great deal of their days lying on their back or in a supine position and consume mostly liquid food. The tendency for liquid food to cause regurgitation, when combined with the pressure lying in a supine position exerts on the lower esophageal sphincter (LES) constitutes a greater risk of reflux acid incidence in infants. The incidence, however, could also be attributed to some other factors like, the anatomy of the infant's stomach, improper or incomplete development of the lower esophageal sphincter during fetal growth, poor diet, overweight, food allergies and a host of other factors.

Because infants, unlike adults, cannot complain of symptoms or explain how they feel, it is always very difficult to know when an infant is suffering from acid reflux. The best bet is to consult a pediatrician. Nevertheless, there are signs and clues that you may look out for in your infant that could suggest the presence of the disease. Some of these clues are:

- Sleeping problems
- Coughing
- Weight loss
- Lack of appetite
- Spitting up frequently
- Apnea
- Unusual irritability
- Chest pain
- Sore throat
- Bad breath
- Crying

Acid reflux disease can also cause respiratory problems including pneumonia, strictures and ulcerations on the esophageal wall, and malnourishment. Although, these signs don't always mean your infant is suffering from acid reflux disease, but they constitute a good enough reason to go see your pediatrician.

Of course, there are a few things you could do to help your child avoid acid reflux. Simple things like changing the child's food, keeping him/her upright for some time, especially after eating, keeping a eye on the child for any sign of chest pain or heartburn and a host of others. This extra attention could be all your child really needs.

Of course, as with any medical condition, there are numerous ways of treating acid reflux, or to give it its full title, gastro-oesophageal reflux disease (GERD). Here, we will look at medication.

Antacids and Alignates

Antacids are generally available without a doctor's prescription and relieve the symptoms of acid indigestion and heartburn (dyspepsia).

The usual ingredients of antacids are magnesium or aluminium and they work by neutralising the acid produced in the stomach which is the cause of indigestion and acid reflux. This group often contain sodium alginate which forms a protective coating over the lining of the gullet and the stomach.

Proton Pump Inhibitors

Proton pump Inhibitors are drugs which shut down the system in the stomach called the proton pump. It is this system which produces acid in the stomach, the leaking of which into the oesophagus causes acid reflux.

These drugs are taken in the form of tablets or capsules, as a powder to be diluted in water or as injections and are used to treat stomach ulcers and a rather rare condition called Zollinger-Ellison syndrome as well as acid reflux.

Omeprazole and Lansoprazole are well known varieties of this particular type of drug although there are several others.

Side effects are rare although they can include diarrhoea or constipation, stomach pains, wind, drowsiness, dizziness and headaches. In unusual cases, a severe allergic reaction can result. My personal experience with Omeprazole was seriously raised blood pressure and severe head pain. However, replacing Omeprazole with Ranitidine and taking blood pressure reduction medication for a couple of weeks sorted out that problem.

H2-Receptor Antagonists

This group of drugs works by attaching to and blocking the H2 receptor area on stomach cells which, when stimulated by the naturally produced chemical, histamine, generates acid.

Ranitidine is the best known of this drug group, others being Nizatidine, Famotidine and Cimetidine.

Side effects are few but can display as diarrhoea, headaches, dizziness and tiredness. Loss of hair has been reported by takers of Cimetidine and with nizatidine, sweating.

Both proton pump inhibitors and H2 receptor antagonists are used together with antibiotics to get rid of Helicobacter pylori infection which can cause stomach ulcers and duodenitis.

Warning: If symptoms of heartburn persist, consult your doctor. They could be indicative of something much more serious than indigestion. Never embark on a course of medication without consulting a doctor. Never take medicines prescribed for somebody else.

An endoscopy is another way of diagnosing acid reflux disease. A tube is swallowed so that doctors can see exactly what is going on inside your body. However, a esophagus of a patient suffering from the disease ordinarily looks quite normal so this method of diagnosis will only directly work for those patients whose esophagus has been damaged by their acid reflux disease. However, if an ulcer or other ailment that can cause similar problems is spotted then it may be possible to discount acid reflux disease.

Heartburn is by far the most common symptom of acid reflux disease and if a doctor suspects your heartburn is cause by acid reflux disease he or she will prescribe you with a medicine that stops the production of acid in your stomach. If it stops then it is assumed that you do indeed have acid reflux disease. This approach of diagnosis can be unreliable.

Acid reflux has forced me to suddenly wake up countless times, in a state of panic, choking on my own acid and gasping for breath.

Nothing can really describe the burning bile feeling, or the taste it leaves in your mouth as you try not to gag and throw-up. I have had to stick a pinch of toothpaste into my mouth and leave it there to try and cut the bile taste.

I was diagnosed with a hiatal hernia as a teenager, which may contribute to acid reflux. The burning acid had left me alone for most of my 20's. However, a high stress job and a subsequent gain in weight and lack of exercise brought the acid reflux back on with a vengeance in my 30's.

When I started having almost constant waves of heartburn and acid indigestion, sitting upright at my desk, I knew it was time to see the doctor.

At that time, I had never heard of acid reflux.

He told me that more than 60 million people suffer from frequent heartburn, acid indigestion or GERD (Gastroesophogeal Reflux Disease).

Acid reflux describes the regurgitation of acid back up the throat. It can also lead to more serious medical conditions that may require hospitalization.

My doctor explained this happens when the lower esophageal sphincter (LES) does not close properly and stomach contents leak back, or reflux, into the esophagus. The LES is a ring of muscle at the bottom of the esophagus that acts like a valve between the esophagus and stomach. The esophagus carries food from the mouth to the stomach.

When refluxed stomach acid touches the lining of the esophagus, it causes a burning sensation in the chest or throat called heartburn. The fluid may even be tasted in the back of the mouth, which is called acid indigestion. Heartburn that occurs more than twice a week may be considered GERD, and it can eventually lead to more serious health problems.

When I finally went and saw my doctor, I was having almost constant heartburn and acid indigestion.

He further explained that symptoms are usually worse at night when the person is lying flat and acid flows easily from the stomach back up the esophagus.

Anyone, including infants, children, and pregnant women, can have GERD.

I was told to make some lifestyle and dietary changes. They included:

1. Using a wedge pillow to elevate my body at bedtime 2. Eating plenty of fiber and drinking lots of fluids. 3. Eating small meals and eating slowly. 4. Not lying down for at least 1-2 hours after eating. 5. Losing extra pounds. Added weight causes extra pressure on your stomach and can irritate symptoms. 6. Wearing loose clothing. Tightly fitting clothes put extra pressure on your stomach. 7. Not exercising for at least one hour after eating. 8. Avoiding the following: alcohol, tobacco, caffeine, chocolate, carbonated beverages, spices, fried and fatty foods, tomato products, citrus, and peppermint.

I am a non smoker and I don't drink that much. I do drink a fair bit of coffee however. The real culprit for me though has been drinking fruit juice of any kind before bed. When I have, my acid reflux has been significantly worse.

As I have made many of these changes to my lifestyle, I have seen some relief.

The elevated pillow works wonderfully. I have also started taking proton pump inhibitor medication, which reduces acid production in my stomach. I have been told surgery may be an option down the road.

Natural stomach acid is in your stomach for a reason. The actual acid is not the cause of acid reflux disease. It is not a nasty poison that someone put there. The acid breaks down foods and sorts them out into the right department so to speak. Proper digestion depends on the presence of adequate stomach acid while you are eating.

When you eat, food moves from the mouth through the esophagus and into the stomach to be digested. At the end of the esophagus, there is a band of muscles that open and allow the food to go into the stomach. It then closes to prevent the food and acid from coming back up into the esophagus. Many people experience acid reflux when foods or digestive juices escape the stomach and come back up through the esophagus.

To make sure your stomach has enough acid at mealtimes, make sure you chew your food thoroughly. Getting saliva mixed into the food as you chew will get the digestion process off to a good start.

Snacking throughout the day causes the stomach to pump out acid gradually instead of saving it for mealtimes when it is needed the most. It is best to not eating closer than three hours apart. For example - if you eat breakfast at six a.m. and lunch at twelve p.m. then a snack at nine a.m. would be fine.

Next, make sure you do not drink too much liquid while eating a meal. Too much liquid dilutes the necessary acid in the stomach. The best policy is to drink a glass of water 30 minutes to one hour before or after your meal, with only a few ounces consumed during the meal. After you have eaten, one hour or two feel free to guzzle down all the pure natural water you want.

Another cause of acid reflux comes from eating large meals. This increases pressure on the stomach causing the stomach contents to be forced out and up into the esophagus. You will need to give your body at least two hours to digest your food properly. In addition, never ever lie down immediately after eating a large meal.

To avoid overproduction of acid eat smaller portions of food at each meal. It would also help a lot to eliminate any fried food or food that contains refined sugar, or caffeine.

Obesity can also increase abdominal pressure, and your risk of suffering from acid reflux as well. If you are overweight, cutting back on the amounts of food that you eat at each meal, and exercising daily should put you on the right path to helping you shed a few pounds.

Acid reflux is caused in some people when the sphincter muscle at the end of the esophagus becomes weak or does not close correctly. Prescription drugs, certain foods, alcohol, and smoking have been known to weaken the sphincter muscles.

Try not to use antacids since they cut down the acids you will need when you eat. Without enough acid, your stomach is unable to do its job of breaking down the food into the various nutrient components.

Inadequate digestion of proteins can cause the liver to increase production of low-density lipoprotein (LDL) cholesterol, which is the bad cholesterol that does the most damage to your body.

Instead of taking antacids, try the following natural approach. If you still have sour stomach in between meals try eating something that will settle it without triggering more acid production such as sauerkraut. Your stomach should relax after eating sauerkraut in five to ten minutes.

The benefits of natural solutions are they normally cost a lot less, but even more important is the fact that natural solutions are a healthier alternative. They tend to reduce the possibility of becoming dependant on addictive chemical drugs.

In fact, in many cases of recurrent acid reflux, lifestyle changes are all that is necessary in preventing heartburn from becoming a chronic disease. Remember to consult a physician before embarking on any treatment strategy.

Whatever you call it, if you are suffering from belching, gas or a bloated stomach lets consider the cause.

Its been a hard and stressful day at work. You race home, gulp down a quick meal and collapse into bed for a well earned rest....But wait.
Suddenly you experience a burning sensation in the top of your stomach.
You may belch and probably feel bloated.
This is the beginning of acid reflux, and can cause acid and stomach content to wash back into the oesophagus creating a burning sensation.

This is often an indicator that something is wrong in your digestive system.
When we eat or drink, the digestive process begins in the mouth with enzymes in saliva beginning the task by starting to break down the food to promote easy digestion.
The food continues down the esophagus, passing through a valve prior to entering the stomach where it comes in contact with stomach (hydrochloric) acid to further break down the food.
If the valve relaxes , or if an excess of acid is produced, acid may be forced back into the oesophagus causing the reflux or heartburn.

(Incidently, I recall cleaning cement from bricks with a commercial hydrochloric acid preparation which indicates the strength of this acid.!)

Occasional reflux is normal, but when it occurs regularly and disrupts daily life, it is described as a disease - Gastro esophageal Reflux Disease, or GERD and can contribute to cancer where the stomach and esophagus meet.

If you are suffering from heartburn the first thing you should do is take a good look at what you are eating.
Over consumption of fatty foods and refined carbohydrates including biscuits, cakes and white bread can affect digestion and and cause reflux symptoms.
Other irritants include salt, spices, sugar, curries, coffee, fizzy drinks and alcohol.

If dietary changes have little or no effect, there are various families of drugs that are available over the counter and on prescription.

PPIs, or proton pump inhibitors, are often prescribed and successful drugs. These work by inhibiting production of the stomach acid that causes acid reflux. They have relatively limited side effects. H2 blockers are a different family of drugs that have the same effect but work in a different way. For the vast majority of long term acid reflux sufferers, PPIs and H2 blockers are a successful drug therapy. Some people, however, fail to respond to treatment with PPIs and H2 blockers and their only option are more radical drugs known as promotility agents. Unfortunately, these can have very severe side effects and are therefore rarely prescribed.

Lastly, for those who fail to respond to drug treatment, or simply want an alternative to the daily inconvenience of taking drugs, surgery is an option. New techniques are constantly being developed, some of which include procedures such as Enteryx Procedure, Stretta Procedure, Nissen Fundoplication and Endocinch.
